When reporting a problem, be prepared to provide the following information:

  • Company Name
  • Contact Name
  • Contact Phone Number
  • Contact Email Address
  • Description of Problem
  • Circuit ID or Equipment ID of Affected Service (When applicable)
  • Approximate Time the Service Disruption Occurred
  • Services Affected

All reported service issues will be assigned a trouble ticket number for tracking purposes. You should record this trouble ticket number before concluding your initial call to report the trouble. Also, please be aware that in some instances you may be required to perform cooperative testing in isolating the trouble.
